We require the following information in order to participate in the food ministry. Please print out the following form, fill in your information, and have it with you when you arrive at the Feeding the Flock building.
If you are unable to print the form, please be prepared to give all of the required information when you arrive.
Please have with you the following items:
1) Food Bank Form
2) Two documents that confirm you are the person registering and are providing your real address (i.e. utility bill, picture ID, etc.). No information on other residents at your establishment is required. We only need information on the person registering.
If you wish to receive diapers, Birth Certificates are required for all children participating.
